시민건강연구소 젠더건강연구센터는 2022년 9월부터 2023년 1월까지 5개월에 걸쳐 회원들과 함께 ‘젠더레짐’을 공부했습니다. 건강에 대한 기존 지식들을 젠더 관점에서 재탐색하기 위한 지적 도구를 쌓기 위한 공동 학습의 과정에서 우리는 건강의 여러 층위, 차원과 상호 교차하는 복잡한 젠더 관계를 건강 체계, 그리고 더욱 너른 건강의 결정요인과 어떻게 연관시키고, 통합시킬 수 있을지 고민해왔습니다. 이 글은 이 질문에 답하기 위한 시작이자, 개념틀로 ‘젠더레짐’의 가능성에 주목합니다. 건강과 보건의료를 포함하여 보다 넓은 영역에서 젠더화된 관계와 역할, 규범, 정동 등의 총체로 나타나는 젠더 구조를 사회적 실재로 포착하고 분석하기 위한 유용한 이론적 개념이나 실천의 틀로 유용하다고 주장합니다. 이 글이 젠더레짐에 대한 더욱 많은 관심과 활발한 논의를 끌어내는 계기가 되길 기대합니다.
모두의 건강을 위한 젠더레짐
이 글은 건강과 젠더를 분석하기 위한 더 넓고 관계적인 관점을 확보하기 위해 ‘레짐’과 ‘젠더레짐’이 사용되는 기존의 방식과 쓸모를 검토한다. 여성 건강과 의료가 계속해서 기술적, 정책적, 제도적 차원의 문제로 축소되고는 경향을 넘어서기 위해서는 젠더 관점의 건강 지식을 실천과 더욱 가깝게 놓기 위한 이론적 도구가 필요하다. 국제정치, 여성학, 사회복지, 사회학 등의 연구에서 논의되는 레짐 개념은 그 정의와 범위, 활용이 다양하지만 총체적이고 실천을 염두에 두는 방식으로 건강과 젠더를 논의할 수 있는 틀이라는 점에서 여러 가능성을 가진다. 고찰을 토대로 우리는 레짐 개념이 모두의 건강과 안녕을 고민하는 성평등한 세계를 모색하는 지식을 쌓아올리는 데에 도움이 될 수 있을 것이라고 주장하였다.
구체적으로 이 글은 총 네 개의 장으로 구성되어 있다. 1장에서는 연구의 배경, 그리고 그 배경이 되었던 공동학습의 목표를 소개한다. 2장에서는 세계보건기구가 제시하는 보건의료체계 구성 요소와 건강 레짐을 비교함으로써 모두의 건강을 위한 총체적 시도로 건강 레짐의 개념적 유용성과 쓸모를 설명한다. 3장에서는 젠더레짐에 대한 문헌고찰 결과를 정리하였다. 젠더레짐의 정의와 쓰임은 국제 관계, 복지국가이론, 여성학 등 학문 영역에 따라 서로 다르다. 따라서 해당 장에서는 젠더레짐의 다양한 의미와 활용을 종합하는 한편, 젠더와 건강을 고민하기 위한 “레짐 접근”의 이론적 가능성을 모색한다. 여기에는 동성 커플의 국민건강보험 피부양자 인정 판결을 분석하는 사례 연구를 포함한다. 마지막 4장에서는 젠더레짐이 보다 넓은 영역에서 젠더화된 관계와 역할, 규범, 정동 등의 총체로 나타나는 젠더 구조를 사회적 실재로써 포착하고 분석하기 위해 유용한 개념임을 주장하며 글을 마무리한다. 이 글이 젠더레짐에 더 많은 관심을 끌어내는 계기가 될 뿐 아니라 젠더와 건강을 형성하는 사회적 실재의 교차성을 껴안는 사회구성체로 젠더레짐을 이해하는데 작은 도움이 되길 기대한다.
Scrutinizing the Gender Regime Concept for Health for All
Gender, as Joan Scott has said, is both a social structure and an analytical category that shapes people’s health. But how does gender integrate into the health system and its broader determinants? How can we relate the multifaceted gender relations that intersect with every dimension and level of health? As we enter the fifth year since the opening of the Gender and Health Research Center, we feel the need to adopt a conceptual framework that can guide our research and activism. In this endeavor, we conducted a literature review on the applications and theoretical possibilities of the gender regime in this report. The paper consists of the following: Chapter 1 introduces the background of the report and explains our purpose for collective learning. In Chapter 2, we compare the health system building blocks with the health regime and explain the conceptual advantage of the health regime in guiding our collective endeavor in health for all. Chapter 3 is our main text, which summarizes the results of our literature review on the gender regime. Definitions and uses of gender regimes differ based on varied disciplines, such as international relations, welfare state theory, and feminist studies. We try to put together the use and meaning of gender regimes in various academic projects and derive the theoretical possibility of a “regime approach” in gender and health. We also include a case study analyzing the recent verdict on administrative litigation about gay couples’ entitlement to dependent relationships on National Health Insurance premiums. Finally, Chapter 4 concludes that gender regimes can be a promising conceptual tool for capturing and analyzing gender structures as social entities, which can manifest the totality of gender relations, roles, norms, and sentiments across a wide range of domains. We hope our investigation can work as an invitation to draw more attention to the gender regime, which we anticipate will develop as a social construct that can embrace the intersecting nature of social reality that shapes gender and health.
